Care Manager Health Plan – CA License Required

Job Summary

A company is looking for a Care Manager, Health Plan with a CA license required for a remote position in the US.

Key Responsibilities:

• Accountable for all Care Management activities for high and moderate members with behavioral health conditions
• Provides telephone triage, crisis intervention, and prior authorizations for select members
• Develops, implements, and communicates patient-centered care plans

Required Qualifications:

• Current licensure as LMSW, LCSW, LMFT, LPC, or RN
• Minimum 5 years’ experience in healthcare, behavioral health, or substance abuse setting
• Knowledge of utilization management procedures and mental health community resources
• Experience in inpatient and/or outpatient settings
• Knowledge of DSM-5 or most current diagnostic edition
