Job Summary

A company is looking for a Care Manager – Complex Care

Key Responsibilities:

• Advocates, informs, and educates beneficiaries on services, self-management techniques, and health benefits
• Develops care plans aligned with physician’s treatment plans and recommends interventions to meet goals
• Coordinates with healthcare team to ensure person-centered care and maximize member health outcomes

Required Qualifications:

• NYS RN license
• Fluency in Spanish, Korean, Mandarin, or Cantonese
• Experience in care management and community health practices
• Proficiency in Microsoft Excel and electronic documentation systems
• Knowledge of relevant product line and managing large caseloads
