Care Manager – Florida Licensed

Job Summary

A company is looking for a Care Manager (RN).

Key Responsibilities:

• Evaluate member needs and develop personalized care plans
• Coordinate with providers and community resources to address member’s needs
• Provide ongoing follow-up, monitoring, and education to members and their families

Qualifications:

• Degree from an Accredited School of Nursing or Bachelor’s degree in Nursing
• 2-4 years of related experience
• Active RN – Registered Nurse State Licensure and/or Compact State Licensure
• Experience working with age group 0-15 months and post-partum preferred
• Reside in the State of Florida
